Submit W-9 Form Online and Save More Than a Stamp!
You save more than a stamp by submitting Internal Revenue Service (IRS) W-9 information to Magellan electronically through www.MagellanHealth.com/provider! Without submitting accurate Taxpayer Identification Numbers (TIN) and certification information to Magellan, you may be subject to back-up withholding taxes. At the direction of IRS law, we must withhold 28 percent of all claims payments made to you unless we have your W-9 information on file. Also, with correct TIN data, we will know how to appropriately direct 1099 reports and other tax-related information to you.
Completing the Magellan online W-9 whenever you experience a change in TIN information gives us exactly what we need to complete our obligations to the IRS and to you. Here’s a list of situations when you should complete the online W-9 form:
- Business (TIN) owner legal name changes (the name on your business tax return)
- You begin to use a new business name different from your legal name
- Your financial address changes
- Your business structure changes, e.g., from Individual/Sole Proprietor to Corporation or Partnership
- You add a new person as user of your business TIN, e.g., a new member of your group practice or rendering provider at a facility or program.

This allows Magellan to enter accurate updates in the provider and claims data systems with the provider’s legal name, TIN and effective date. And information submitted online is routed electronically and rapidly to the appropriate Magellan departments.”
Directions for Submitting the W-9 Form Online
Just sign in at www.MagellanHealth.com/provider. From your My Practice page, choose “Display/Edit Practice Information” from under “My Practice.” The “W-9 Form” can be found there with a “Submit Online” button. It’s fast and just as easy as completing a hard copy W-9 form.
