Michigan Treatment Request Form Submission
To reduce turnaround times for authorization decisions, Magellan encourages providers to submit treatment request forms (TRFs) online. The process is easy, efficient, and environmentally friendly.
To submit online TRFs, sign in to the secure section of this provider website with your username and password. After sign-in, select My Authorizations/Get More Outpatient Authorizations and follow the form instructions.
If you must submit TRFs by fax, please note that the fax number for submitting TRFs to the Michigan Care Management Center changed in March. The new number is: 1-800-501-0185.
This fax number is dedicated to TRFs, so please do not fax TRFs to any other number or fax other documents to the TRF number. We plan to disable the previous fax numbers by fall 2009.
We understand that prompt authorizations are important to you and your clients. To help avoid errors that often delay the authorization process, we have provided a list of the most common mistakes found on hard copy TRF forms submitted by our providers.
These issues are as follows:
- Wrong member DOB
- Wrong member ID number
- Wrong patient listed
- Missing provider name and address
- Missing provider signature
- Missing provider practitioner ID number
- Providers writing over typed information (e.g., changing doctor’s names, crossing out old addresses or changing the authorization number).
Again, if you absolutely cannot submit your TRFs online, here are some tips to enable timely TRF authorization decisions:
- Use the TRF included with the authorization letter (The TRF form included with authorization letter is coded for the particular member and episode of care. Subsequently, copying a TRF from one authorization letter to use for another member will delay the authorization process.)
- Write legibly and don’t write over pre-populated information on the form
- Include all identifying patient and provider information
- Sign the form!
