New Consolidated EASI Form Now Available

As a result of input received from EAP providers, and from our client companies and organizations who purchase EAP services, Magellan recently streamlined the Employee Assistance Service Information (EASI) form, the document that EAP providers complete for each member they treat. We reduced the overall number of questions on the form from 18 to 13.

The primary focus of the EASI form is to measure EAP interventions, outcomes and effectiveness. It also allows us to collect aggregate statistical information regarding problem areas addressed within the EAP, which is critical for identifying trends and for future planning.

“We listened to the feedback from our veteran EAP providers who said they would appreciate any effort to streamline their administrative paperwork,” said Christine Leslie, director of EAP Affiliate Management. “We think the new form provides critical information to our corporate customers regarding the value of their EAP, while reducing the administrative time EAP providers need to spend getting that data to us.”

The EASI form is on this provider website, under the top-menu item “Forms.” (Choose “EAP Forms.”)
