Psychological Testing Request Form Revised

To improve clarity and ease-of-use, Magellan made minor modifications to our Request for Psychological Testing Preauthorization form. Although some questions are now more concise, the process remains the same.
 
In most cases, an initial assessment (90801) by a behavioral health provider must be completed before psychological testing will be authorized. To avoid potential issues with reimbursement, do not initiate psychological testing until an authorization is received.

Send the completed psychological testing request form to Magellan at the address or fax number located on previous authorization correspondence you may have received for the member. You also can obtain the proper address or fax number by calling the phone number on the back of the member’s benefit card.

The revised Request for Psychological Testing Preauthorization form and instructions are available at this website by clicking on Forms, then Clinical Forms. Providers without Internet access may call the appropriate Magellan Care Management Center to request the form.
