Direct Deposit Available to EAP Providers

Access EFT Form Online

EAP providers now can take advantage of electronic funds transfer (EFT) for EAP payments. Just complete and submit the EAP EFT form posted on this provider website to request that we directly deposit payments for EAP services to your business bank account. (Under Providing Care, choose EAP Reimbursement, then EAP EFT form).

EFT is quicker than the standard process of mailing and cashing or depositing a check, leaving you more time to devote to your practice.

In order to receive electronic funds transfers in lieu of a paper checks, you must have a Form W-9 on file with Magellan and be the owner of the Taxpayer Identification Number (TIN) under which accounts payable invoices are paid by Magellan. Please print legibly and include a valid e-mail address to which you want us to send EFT remittance notifications. If any changes occur to your information, simply complete and submit a new form.

Complete the EAP EFT form today and then fax, mail or e-mail it to us to begin taking advantage of this simple, secure and efficient method of receiving payments.
