Consumer Health Inventory (CHI)
The CHI is a self-report measure that assesses functional health status, strengths, symptoms, substance use and recovery progress. It is used with individuals age 18 and older. In collaboration with Quality Metric, Magellan developed the CHI specifically for use with public-sector/public-program populations, incorporating items from the SF-BH.
Both caretaker and provider reports are generated immediately to print and use in treatment planning and monitoring of progress over time. Provider reports are also available to view any time on this website on the My Practice page after secure login, under the View Outcomes Reports link.
