SF-BH™
The SF-BH™ is a functional health assessment tool used for members age 14 and older to measure health and wellness at different points in time. This tool enables members to self-report their mental and physical health status, and have real-time outcomes summary reports available to themselves and their providers to monitor progress.
It is based on QualityMetric’s SF-12® Health Survey, which is nationally recognized as a leading health assessment tool with a general health focus.
