Provider Data Change
Update name, address and other practice
information
I wanted to let you know
that I've changed my (address, phone number, email
address, credentialing status, etc.).
Thank you for your email. If you are an in-network
provider, you can now submit changes to your practice
information online. Simply go to www.MagellanHeatlh.com,
click on I'm a Provider, and Sign-in in the sign-in
box on the right hand side of the screen. If you've
never had a login username and password for Magellan
before, click on New User and follow the onscreen
instructions to obtain a username and password.
Once you are logged in, click on Display/Edit Practice
Information in the left-hand menu under the sub-heading "My
Practice." Then follow the on screen instructions
to submit your changes.
If you are not a credentialed and contracted Magellan
provider, you can access a Provider
Data Change form on our website by clicking
the link below. Complete the form indicating your
changes and submit it to the address indicated on
the form.
