Sharp Health Plan

Effective Jan. 1, 2026, Magellan* will no longer manage the behavioral health services for Sharp Health Plan members.

To continue as an in-network provider serving these members on and after Jan. 1, 2026, you’ll need to follow Sharp Health Plan's processes to become a contracted provider. Sharp Health Plan is transitioning to OptumHealth Behavioral Solutions of California; contact the Optum Provider Service Line at 1-877-614-0484. 

For dates of service through Dec. 31, 2025, continue with current processes via Magellan. For dates of service on and after Jan. 1, 2026, verify eligibility and benefits, obtain prior authorization, and submit claims following Sharp Health Plan's/Optum's processes.

Behavioral Health Injectable/Infusion Prior Authorization Request Form (PDF)

  • To request authorization for an injectable or infusion medication, complete the authorization form above. 
  • Fax the form along with applicable medical records to Sharp Health Plan at (858) 636-2426. 
  • Sharp Health plan will review for medical necessity and notify the provider and member of the determination.
  • Sharp Health Plan will also notify Magellan of the prior authorization request and determination.

Referring Members to In-Network Labs

Laboratory expenses are one of the biggest contributors to a patient’s healthcare costs. That’s why Magellan* and our health plan client, Sharp Health Plan, ask providers to refer members to an in-network provider.

Sharp Health Plan members who require outpatient lab work associated with mental health and/or substance use disorder treatment must be referred to a lab contracted directly with their health plan. In-network labs include:

  • Laboratory Corporation of America (Labcorp)
  • Pacific Rim Pathology
  • Quest Diagnostics

If you or a Sharp Health Plan member has any questions about coverage for outpatient lab services associated with their mental health and/or substance use disorder treatment, contact Sharp Health Plan Customer Care at 1-858-499-8200. Note: All genetic and pharmacogenetic testing requires prior authorization from Sharp Health Plan.
